Individuals working with animals are required to enroll in the program and complete the baseline screening questionnaire. The new questionnaire will be online the first week of September. In order to upgrade the survey and improve the process, individuals completing the updated health survey for the first time will be required to provide some of the same health information which was previously requested in the prior version of the on-line questionnaire. However, besides some general health questions and allergy-related questions, other questions asked will be species or health risk specific. During the protocol renewal process, individuals will be asked to update their health information only. All health information is completely confidential and contained in a secure database. Information is reviewed only by CDP clinical personnel. Recently the Federal Aviation Administration (FAA) identified regulatory deficiencies in the UCSF Safe Shipping Program related to laboratory shipments of infectious substances as well as dry ice. Individual PIs may be subject to fines as high as $500,000 as well as jail time for non-compliance with federal shipping regulations.
