| THE
OFFICE OF RESEARCH |
| IT services are provided to the Office
of Research through two distinct groups. The Office of Academic and
Administrative Information Systems (OAAIS, formerly ITS) handles domain
accounts, email services and network/desktop support. The IT team
of the Office of Research (now also a part of OAAIS) creates and supports
custom software such as RIO (Research Information Online), Research
Online and RITS. These two groups each have their part in activating
or terminating the accounts of employees of the Office of Research. |

| The most important thing to remember
when requesting a new account or terminating an existing account,
is that the supervisor or manager must make these requests. For security
reasons, requests made by the employee will not be honored. |

| New Employees in the Office of Research |
- Create a Remedy request either online at www.help.ucsf.edu or by calling 514-4100, option 2.
- You will need to include several items in the Remedy ticket
details.
- The name of the New Employee.
- The accounts requested: domain, email or both.
- Initial group assignment of the Remedy ticket should be {ITS-CSSP-DLS}. They will create the domain
and email accounts.
- If permissions are needed to any Office of Research custom software
(RIO, Research Online, RITS, etc.), include
the application and the permission rights needed in
the Remedy ticket details. Please be specific and state
the type of tasks to be performed or the role to be assigned. If the new employee
is replacing someone or will need the same rights as
another employee, you can state "grant permissions similar
to 'Other Employee'". Also, mention the Office
of Research Services {ORS} in the details so when the CSSP-DLS team has completed their tasks,
they can re-assign the Remedy ticket to the Research IT group.
- Complete the top section of the 'New Account' request form located here.
Certain fields are required. You will need the new Employee's UCSF ID and a Fund/DPA, among others, to complete this request.
- When the Remedy ticket is assigned you will be contacted to provide the 'New Account' request form.

We will need a domain and email account for 'New Employee'.
In ORS, 'New Employee' will need access to the 'Authorization' and 'Contact'
sections of RITS, the 'Training Attendance' sections of Research Online,
and will need 'IACUC Admin' rights in RIO. |

In ORS, 'New Employee' will need RITS, Research Online, and RIO permissions
similar to 'Other Employee'. |

| Employees leaving the Office of Research |
- Create a Remedy request either online at www.help.ucsf.edu or by calling 514-4100, option 2.
- You will need to include several items in the Remedy ticket
details.
- The name of the Employee.
- The accounts that need to be closed: domain, email or both.
- Initial group assignment of the Remedy ticket should be {ITS-CSSP-DLS}.
- If permission rights should be revoked in Office of Research custom software, mention the Office
of Research Services {ORS} in the details so when the CSSP-DLS team has completed their tasks,
they can re-assign the Remedy ticket to the Research IT group.
- Complete the left section of the 'Account Termination' request form located here.
This form allows the manager or supervisor to specify what is to be done with the employee's emails and data.
- When the Remedy ticket is assigned you will be contacted to provide the 'Account Termination' request form.
No data will be deleted until this form has been completed and submitted.

| Note: In the case of an immediate termination, please call Kurt Glowienke
at 476-9807 and request the employees' domain account be immediately disabled. |

Data and email disposition instructions will follow shortly.
In ORS, 'Employee' will no longer need access
to any Office of Research applications. |
