1. Fellowships

Signed by Fellow (if available) and Sponsor. Chair signs only if backup required by department.

2. Confidentiality Disclosure Agreement (CDAs), Material Transfer Agreements (MTAs), and Intergovernmental Personnel Act Agreements (IPAs)

PI Signature only.

3. Contract, Grant, Subcontract and Unfunded Research Collaborations (URGs)

* Effective 3/15/99, the Dean's signature is no longer required on the C&G Approval Form for the School of Medicine (except for SFGH, Fresno, and VAMC locations as noted below). The C&G Approval Form should either indicate N/A on the Dean's line or be left blank.

‡ If the project is located at Fresno, or the VAMC, then the appropriate designated official at the site will sign. The signature of the Dean of the School of Medicine on the main campus is not required.

† If the project is located at SFGH the designated SFGH Dean's Office official will sign except for Department of Medicine-SFGH. Authority to sign Department of Medicine-SFGH forms has been delegated to the department chair, Dr. Talmadge King. The signature of the Dean of the School of Medicine on the main campus is not required.

** Effective 5/17/00, the Dean's signature for Stomatology has been delegated to the Chair.

± Effective 9/28/01, the signature authority for LPPI and the Department of Psychiatry has been delegated to Craig Van Dyke, Director, LPPI, and Chair of the Department of Psychiatry.
